Palliative Care

Palliative care helps people manage their serious or chronic illness or disease. The care team helps the patient feel better by preventing or treating the symptoms and side effects of their disease and treatment. Patients can continue to receive treatment intended to cure their illness. Care is provided by a specially trained team of doctors, nurses, social workers, and other specialists who work together. Visits are typically once every two weeks but are based on patient needs and may be more or less often. Palliative care is based on the needs of the patient, not on the patient’s prognosis. Palliative care is covered by most insurance. Copays may apply.

While it sounds like this would be “religious” counseling, it isn’t.  Spiritual pain is the distress you feel if you struggle to find purpose, comfort, and even connections to others.  Emotional and spiritual care is an important part of chronic illness.  It can help patients and families find meaning, hope, and peace in their journey through their illness.
Medication Management

Sometimes coordinating medications is hard.  Names that are too long and sound alike, knowing when to take which medication, and then you come out of the hospital and things change.  We will help you understand your medication schedule, set you up with a deliverable pharmacy, and/or teach you or your caregiver how to set up a medication box.

Our services are available to you, irrespective of your insurance coverage or financial situation. Palliative care is typically covered by most insurances, although copays may apply.

Palliative care is appropriate at any age and at any stage in a serious illness and can be provided together with curative treatment. Medicare and Medicaid cover palliative care patient visits by a physician or advanced practice registered nurse (APRN). These visits may require copays. Some Medicare Advantage and Commercial Plans offer coverage for palliative care visits and services, but specifics vary by plan.
